Your safety is of utmost importance to us. For that reason we need you to please read through this Medical Questionnaire before you join us on one of our snorkeling trips. We would like you to do this in order to establish that you are healthy enough snorkel with us.

The purpose of this Medical Questionnaire is to find out if you should be examined by your doctor before participating in recreational snorkeling. You don’t need to sign the form now - you are only required to read through it to establish if you will need to consult a doctor. We however require that you sign the form prior to your snorkeling trip (you can do this at our Scuba Dive Centre in Cape Town).

 A positive response to a question does not necessarily disqualify you from snorkeling. A positive response only means that there may be a pre-existing condition that may affect your safety whilst snorkeling, and that you must seek the advice of your physician prior to engaging in in-water activities.

So, if your answer to every question is negative, then you can get started with your snorkeling. If you should answer positively to any question, please first consult your doctor, before attending a snorkelling with seals trip.
